POLICY
FOR RETIREMENT FROM THE PRACTICE OF DENTISTRY OR DENTAL HYGIENE
IN THE STATE OF OHIO
Preamble:
The purpose of this policy is to address what is required for
a dentist or dental hygienist who wishes to retire his or her
license to practice in the state of Ohio.
The procedure for a dentist wishing to temporarily retire his/her
license to practice dentistry in the state of Ohio subsequent
to suspension for failure to renew is addressed in section 4715.14(B)
of the Ohio Revised Code.
Any dentist or dental hygienist who wishes to retire his/her license
to practice in the state of Ohio may do so according to the following
stipulations:
- All
requests to retire must be submitted to the Board in writing;
- Biennial
Renewal/Registration fees will be waived, provided that the
licensee is currently registered and in good standing with the
Board;
- Once
retired, the practitioner may not practice in any capacity,
including that of insurance consultant, during the time the
license is in retirement.
If
the license has been in retirement longer than one year, and the
dentist or dental hygienist wishes the license to be reinstated,
it will be necessary for the applicant for reinstatement to do
the following:
Complete a reinstatement application and pay the required fee;
- Provide
proof of having met all necessary continuing education requirements;
- Provide
proof of immunity to or immunization against the Hepatitis B
virus;
- Appear
before the Board for a reinstatement interview;
- Take
the jurisprudence examination.
- Successfully
complete any remediation the Board may deem necessary.
Once
these requirements have been met (no exceptions), the Board may
consider the applicant's request for reinstatement.
